DIVA is a pragmatic randomized clinical trial (RCT) to determine: among (P) preterm infants born 24-27 6/7 weeks gestation undergoing extubation from mechanical ventilation, whether (I) Non-invasive neurally adjusted ventilatory assist (NIV-NAVA) (C) compared with Non-synchronized nasal intermittent positive pressure ventilation (NS-NIPPV), will reduce the incidence of (O) extubation failure within (T) 5 days (120 hours) of extubation.

The objective of this study is to investigate the mechanisms of respiratory functional and respiratory motor responses to the scES as an important step toward the long-term goal of developing effective evidence-based rehabilitation strategies for patients with SCI. The central hypothesis is that scES raises the excitability of motor networks for respiration resulting in amplified use-dependent neural plasticity in response to the respiratory training. The rationale for the proposed study is to justify the development of a new direction in respiratory rehabilitation by using respiratory training in combination with electrical spinal cord stimulation.

Are you 75 years old or older and interested in participating in a clinical trial where researchers are studying whether statins, which are often used to treat high cholesterol, can help improve heart and brain health. To participate, you cannot have cardiovascular disease or dementia. Have you or your child been diagnosed with a blood disorder? If so, you (or they) may be able to participate in a clinical study where researchers are studying a type of infusion using cord blood. Participants can be of any age but must have a blood disorder diagnosed by their doctor. Are you scheduled for a stem cell transplant with cord blood? This study will assess the safety and effectiveness of certain cord blood transplants. The study will help researchers learn the best methods for collecting, storing, and using cord blood in transplants. To participate in this study, you must have a disorder that compromises your body’s ability to make blood cells. This study is located in Bethesda, Maryland.

Have you been diagnosed with peripartum cardiomyopathy and are within 5 months of your due date? This study is evaluating the effectiveness of a medicine called bromocriptine to treat peripartum cardiomyopathy, or weakness of the heart muscle that occurs during or shortly after pregnancy. To participate in this study, you must be at least 18 years old, diagnosed with peripartum cardiomyopathy, and within 5 months of your delivery date. This study takes place in Pittsburgh, Pennsylvania.

This is a multi-center study enrolling patients suspected or newly diagnosed with myelodysplastic syndromes (MDS), myelodysplastic syndromes/myeloproliferative neoplasms (MDS/MPN) overlap disorder, or idiopathic cytopenia of undetermined significance (ICUS). Participants will be followed long term. Clinical data, blood, and tissue samples will be collected to establish a biorepository to facilitate the study of the natural history of MDS.

Are you in need of a heart or lung transplant or have you had one in the past 3 months? Have you undergone a lung or heart transplant in the past 3 months or are you a candidate for heart or lung transplant? Some people with advanced heart and lung disease have heart and lung transplants but these new organs are often rejected. When this happens, patients must have repeated biopsies, which are invasive and expensive. Researchers in this clinical study want to see if a blood test can predict organ rejection and take the place of biopsies. To participate, you must be 18 to 80 years old and be a candidate for heart or lung transplant or have had a heart or lung transplant no more than three months ago. The study is taking place in Bethesda, Maryland, at the NIH Clinical Center, as well as in Baltimore, Maryland, and Falls Church, Virginia.

Are you at least 18 years old and have been told by your doctor that you need a procedure called transcatheter aortic valve replacement? This study will test a new device designed specifically for use during a procedure called transcatheter aortic valve replacement. Researchers want to make transcatheter aortic valve replacement safer. To be in the study, you must be at least 18 and need a transcatheter aortic valve replacement. The study will take place in locations in California, Georgia, Massachusetts, New York, Virginia, Washington, and Washington, D.C.
