Each day, participants who took their medicine will be eligible to win a prize through a smartphone app that tracks whether they are taking their medicine. Are you a professional caregiver for an adult who has serious heart failure? This study is testing whether electronic educational resources, in addition to in-person training , help home aids be more knowledgeable and confident in caring for adults who have serious heart failure. To participate in this study, you must be between 18 and 75 years old, be employed at Visiting Nurse Service of New York's Partners in Care agency, and care for an adult who has heart failure. This study uses positron emission tomography/magnetic resonance imaging (PET/MRI) to look at the relationship between psychosocial stress and systemic inflammation/atherosclerosis. Are you an adult with atrial fibrillation who recently had a catheter ablation procedure? This study will investigate the effectiveness of an implantable cardiac monitor-guided device to monitor participants for recurrent atrial fibrillation following ablation. Are you an adult with cardiomyopathy who is eligible for a left ventricular assist device (LVAD)? This study is investigating whether a ventricular arrhythmia (VTA) ablation at the same time that an LVAD is implanted lowers the risk of VTA after surgery. Are you an adult who has received urgent care for moderate to serious COPD in the past 4 weeks? This study aims to find out whether a method called Comprehensive Health Informatics Engagement Framework for Pulmonary Rehabilitation can help people who have COPD stick to their treatment plan. Are you a 30- to 55-year-old smoker or non-smoker who would like to support better understanding of COPD risk factors? This study aims to find early signs of COPD in adults who have a high risk of this condition. Researchers will compare results from questionnaires, lung tests, and other tests in people who smoke and in people who do not smoke to look for factors that predict which people will develop COPD.
