The goal of this mechanistic clinical trial is to learn about the effects of medications called soluble guanylyl cyclase stimulators on vascular function and markers of kidney and brain injury in patients having heart surgery. This will provide important information to determine the underlying reasons that patients have some kidney and brain function problems after having heart surgery.

Are you scheduled for open-heart surgery for your aortic valve? This study aims to look at changes in inflammation and gene activity while surgery patients are on a heart-lung bypass machine. To participate in this study, you must be at least 20 years old and be scheduled for aortic valve surgery, either with or without coronary artery bypass grafting (CABG). This study is located in Boston, Massachusetts.

Did you develop atrial fibrillation after coronary artery bypass graft (CABG) surgery? Did you develop atrial fibrillation after a coronary artery bypass grafting (CABG) surgery? This study is looking at how well oral anticoagulation medicines work to prevent complications, such as blood clots and heart attack, for patients who experience post-operative atrial fibrillation (POAF). Are you planning to have coronary artery bypass grafting (CABG) surgery to treat your heart disease? This study aims to use computed tomography (CT) imaging to help doctors and their patients make better decisions about heart surgery. Do you or your child have dyslipidemia? This study is exploring how different diagnostic tests can help us understand how lipid disorders, including high blood cholesterol and high blood triglycerides, affect the body. Information from this study may help improve the way lipid disorders are diagnosed or treated in the future. Participants in this study must be at least 2 years old. Are you an adult who is healthy or has problems with the level of cholesterol or other lipids in the blood? This study is gathering information on the risk factors for very high or very low levels of lipids, or fats, in the blood. Researchers will collect participants’ demographic information, medical history, and blood samples. To participate in this study, you must be at least 18 years old. This study takes place in Dallas, Texas.

Ventilated pediatric patients are frequently over-sedated and the majority suffer from delirium, a form of acute brain dysfunction that is an independent predictor of increased risk of dying, length of stay, and costs. Universally prescribed sedative medications-the GABA-ergic benzodiazepines-worsen this brain organ dysfunction and independently prolong duration of ventilation and ICU stay, and the available alternative sedation regimen using dexmedetomidine, an alpha-2 agonist, has been shown to be superior to benzodiazepines in adults, and may mechanistically impact outcomes through positive effects on innate immunity, bacterial clearance, apoptosis, cognition and delirium. The mini-MENDS trial will compare dexmedetomidine and midazolam, and determine the best sedative medication to reduce delirium and improve duration of ventilation, and functional, psychiatric, and cognitive recovery in our most vulnerable patients-survivors of pediatric critical illness.

Is your newborn in the NICU at Holtz Children’s Hospital in Miami? Newborns born very prematurely often need oxygen therapy or ventilation to help them breathe and survive. This study will help doctors understand how changes in oxygen and carbon dioxide levels while newborns are getting treatment affect how their lungs develop. While your newborn is in the newborn neonatal intensive care unit (NICU), researchers will record his or her oxygen and carbon dioxide levels, heart rate, and other measures. To participate in this study, your newborn must have been born prematurely between 23 and 28 weeks’ gestation, be less than 28 days old, and be receiving oxygen therapy. This study is located in Miami, Florida.
