The Investigators propose a randomized trial of smart texts versus usual care for 6 months. The Investigators will randomize 100 PAH patients to the mHealth intervention or usual care. The Investigators will test the effect of a text-based mHealth intervention on HRQOL in PAH using the PAH-specific emPHasis-10 questionnaire. The Investigators will also test the effect of an mHealth intervention on exercise capacity, measured by a supervised home-based 6MWD test. Finally, the Investigators will examine the effect of the intervention on time to clinical worsening (composite of PAH therapy escalation, PAH hospitalization, and death) one year after randomization.

Are you a healthy adult who wants to join a study about less sitting? You might qualify for this research. Researchers are testing if walking breaks can improve cardiovascular health, like blood pressure and glucose (sugar) levels. To join, you must be 18 years old and have no long-term health issues or not be taking medications for blood pressure or diabetes. Participants will have to go to the study site for a 9-hour visit and walk on a treadmill during the day. The study is taking place in New York City, New York.

Are you a young adult who was born prematurely? Are you a young adult who was born prematurely? Researchers are trying to learn about how salt in the diet influences blood pressure in young adults who were born prematurely. You may be able to participate if you are between 24 and 32 years old and were born at less than 34 weeks. People who were born at full term may also be able to participate. The study is taking place in Winston-Salem, North Carolina.

Do you have high blood pressure? In this clinical trial, researchers are studying different ways to improve home blood pressure monitoring. You may be able to participate if you are between 18 and 85 years old and diagnosed with high blood pressure. Participants will work closely with clinicians or other healthcare providers to schedule appointments, coordinate care, measure blood pressure, answer your questions, and treat your high blood pressure. The study is taking place in Rochester, New York.

Researchers want to learn more about how well people with pulmonary arterial hypertension are able to exercise. To participate, you must be 18 years old and be able to walk for at least 6 minutes. The study is taking place in Nashville, Tennessee.

Does your child or someone you know have pediatric pulmonary hypertension? Does your child or someone you know have pediatric pulmonary hypertension? This study is using a wearable monitoring device to promote physical activity in a 16-week intervention. To participate in this study, the child must be 8 to 18 years old and have a Wi-Fi-enabled mobile device in the family, capable of receiving text messages. This study is located in Philadelphia, Pennsylvania.

Do you have pulmonary arterial hypertension? Do you have pulmonary arterial hypertension? Increasing physical activity is helpful for people with pulmonary arterial hypertension. Researchers are testing whether sending text messages is an effective way to encourage people with pulmonary arterial hypertension to be more physically active. You must be 18 years old to participate. The study is taking place in Nashville, Tennessee.

Have you lived in Alabama, Kentucky, Louisiana, or Mississippi for at least two years and are you interested in participating in a clinical study where researchers are trying to better understand health and disease in rural areas? The study team will track your physical activity using a provided Fitbit device and will also ask questions about your health and lifestyle through a cell phone app. You will also receive a kit to measure environmental and health factors in your homes. Participants must live in one of these counties: Dallas County, Alabama; Wilcox County, Alabama; Boyle County, Kentucky; Perry County, Kentucky; Garrard County, Kentucky; Breathitt County, Kentucky; Assumption Parish, Louisiana; Franklin Parish, Louisiana; Oktibbeha County, Mississippi; and Panola County, Mississippi.
