Do you have heart failure and depression? This study aims to see whether stepped care for depression helps people who have heart failure manage their care (follow heart-healthy lifestyle recommendations and take their prescribed medicines). Participants will either receive usual care for depression or take part in cognitive behavioral therapy, where they work with a mental health counselor to learn ways to manage symptoms and cope with challenging situations. Certain participants may then be referred for antidepressant medicines. All participants will then receive heart failure education. To participate in this study, you must be at least 25 years old and diagnosed with heart failure and major depressive disorder. This study is located in Saint Louis, Missouri.
