Does your developing baby have a heart defect? Have you had a stillborn baby? This study tests a new technology to measure fetal heart activity and find possible problems early. These problems include fetal arrhythmia and conduction disorders such as Brugada syndrome. To participate in this study, you must be at least 18 years old and pregnant, and have one of five high-risk complications of pregnancy: a fetus with a major congenital heart defect, hydrops, or gastroschisis; a previous unexplained stillbirth; or twins who share a placenta. This study is located in Madison and Milwaukee, Wisconsin.

Have you been diagnosed with orthostatic intolerance? This study aims to find out how the body’s regulation of basic functions, such as heart rate and blood pressure, is altered in people who have orthostatic intolerance. This condition, which has an unknown cause, is characterized by a racing heartbeat, dizziness, lightheadedness, and other symptoms that occur when a person stands up. To participate in this study, you must be between 18 and 80 years old and have orthostatic intolerance. This study is located in Nashville, Tennessee.

The investigator will study the origin of fungal translocation in HIV, its relationship to the mycobiome, and its relationship to lung function and inflammation. Supported by the preliminary data and published studies, this project is based on the premise that circulating BDG derived from microbial translocation stimulates inflammation and worsens lung function in PWH.

Chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death in the United States. Current treatments for COPD focus on inhaler therapies that do not address manifestations of the disease on other organ systems. Platelets, which are small blood cells that typically help with clotting, are also involved in generalized inflammation and dysfunctionality of immune cells when these cells become activated.

A randomized controlled trial of a food delivery dietary intervention targeting increased omega-3 intake to determine whether dietary modifications can improve Chronic Obstructive Pulmonary Disease (COPD) outcomes and attenuate the adverse effects of particulate matter on respiratory health.

Investigators believe that study results will comprehensively address the impact of an evidence-based nutrition intervention on COPD health and provide a framework for dietary intervention within other chronic diseases disproportionately impacting susceptible, low-income populations.

This is a pilot study on the impact of switching from cigarettes to Electronic Cigarettes (EC) on disease-related clinical symptoms and biomarkers of harm in smokers with preexisting Chronic Obstructive Pulmonary Disease (COPD). The researchers hypothesize that the smokers who switch to EC completely or significantly will experience reduced COPD symptoms, risks of exacerbations, and decreased levels of oxidative stress and inflammation.

Pulmonary rehabilitation (PR) is a structured program of exercise and self-management support that has been proven to relieve shortness of breath and increase quality of life when initiated after an exacerbation, but unfortunately, few eligible patients participate. This project will compare the effectiveness of two novel strategies - one involving video narratives of other patients telling their story of how they overcame challenges and completed PR, the other involving telephonic peer coaching with an individual with lived experience - to enhanced usual care, and to each other, at increasing patient participation in PR after an exacerbation.

Chronic obstructive pulmonary disease (COPD), one of the leading causes of death in the US, disproportionately affects low socioeconomic communities. While few interventions effectively modify the course of COPD and improve outcomes, pulmonary rehabilitation is the one notable exception. However, implementation of this resource-intensive program in real-life settings, and in particular, for underserved communities, has proven to be challenging. Safety-net centers that serve primarily under-insured populations lack financial resources to provide pulmonary rehabilitation.

The 10-week COPD Wellness and Plus+ Program directly addresses this gap, and yet, programs like these do not automatically lead to improved outcomes, which leads to the implementation of a Health Advocates program to address participant's social needs and barriers to healthcare.

Do you or your child have a lung disease? This study will investigate the genes involved in the breathing process and in the development of lung diseases such as asthma or sarcoidosis to improve understanding of the role they play. To participate in this study, you or your child must be between 2 and 90 years old. This study is located in Bethesda, Maryland, at the NIH Clinical Center.

Do you have chronic bronchitis and COPD and want to improve treatment? This study will be testing how well a medicine called ivacaftor that is used to treat cystic fibrosis works also to treat symptoms of chronic bronchitis. New research suggests chronic bronchitis may be due to a problem similar to cystic fibrosis. To participate in this study, you must be 40 to 80 years old, have a history of smoking, and have not been diagnosed with asthma or cystic fibrosis. This study will be conducted in Birmingham, Alabama.
