Community Health Worker Toolkit

Community health workers (CHWs) play an important role in educating their communities about heart disease by implementing culturally tailored health promotion and disease prevention programs. Find resources designed to help CHWs bring science-based, easy-to-understand information on heart health to community members in an engaging way. These heart health toolkits are tailored for several different ethnic communities including African Americans, Hispanics/Latinos, American Indians, Alaska Natives, Asian Americans, Native Hawaiians and other Pacific Islanders. As trusted members of their community, CHWs can share this information and encourage healthy lifestyle behaviors that can help reduce health disparities.

#OurHearts Toolkit

February is American Heart Month. During February and beyond join us in raising awareness about heart health. We offer a variety of educational materials and resources to use in your community, on social media, and more that help people learn about the importance of protecting their hearts. Also, find National Wear Red Day® resources. Join NHLBI, The Heart Truth®, and organizations around the country on National Wear Red Day, celebrated each year on the first Friday in February, to bring greater attention to heart disease as the leading cause of death for Americans.

National High Blood Pressure Education Month Resources

May is National High Blood Pressure Education Month. Through its research and clinical trials, the NHLBI is improving the health of people who have high blood pressure. Join us in raising awareness about high blood pressure. We offer educational resources for health professionals and patients to help people manage and lower their blood pressure, including DASH eating plan resources.
