PHA Offers Tools to Assist Physicians in Helping Patients

The Pulmonary Hypertension Association’s comprehensive Empowered Patient Toolkit can make visits with your patients more productive. When they track their medical history, medications and medical visits, and record their symptoms and questions for you between visits, you will both get the most out of the time you spend together. Refer them to www.PHAssociation.org/OnlineToolkit.

The Pulmonary Hypertension Association wants to help you help patients. We offer three distinct packets: for patients (Envelope of Hope), caregivers (Resources for Family & Friends) and family members of children with PH (Resources for Family & Friends: Pediatric). Clinicians may Request referral postcards from the Free Materials section of PHA’s Online Store, www.PHAssociation.org/store/freeform.aspexternal linkor by emailing EoH@PHAssociation.org.
