to -4.5%) from 1991 to 1997 and 2.1% (95% CI -2.5% to -1.8%) from 1998 to 2003
among men. The respective changes among women were -5.2% (95% CI -5.8% to -4.7%)
and -2.1% (95%CI -2.7% to -1.5%).
