ATP III Update 2004: Implications of Recent Clinical Trials for the ATP III Guidelines

The Adult Treatment Panel III (ATP III) of the National Cholesterol Education Program issued an evidence-based set of guidelines on cholesterol management in 2001 (Executive Summary published in JAMA, 2001;285:2486-2497). Since the publication of ATP III, 5 major clinical trials of statin therapy with clinical end points have been published. These trials addressed issues that were not examined in previous clinical trials of cholesterol-lowering therapy. The present document reviews the results of these recent trials and assesses their implications for cholesterol management.

Healthy Weight Community Outreach Initiative Strategy Development Workshop Report

The NHLBI Obesity Education Initiative (OEI) convened a 2-day meeting to help develop a national public education outreach initiative to help reduce and prevent overweight and obesity in the United States. This Strategy Development Workshop, held on February 17-18, 2004, convened more that 70 public health leaders, nutritionists and dieticians, food industry experts, health communicators, youth marketing experts, park and recreation officials, and others from professional societies and constituency groups. The Workshop proceedings illuminated the complexity of the factors leading to weight gain; the many environmental and societal influences on the eating and physical activity habits of various population groups; and the community-based strategies that might encourage healthier habits.

Clinical Alert: Transfusions Should Continue in Children with SCD who are at Risk of Strokes

The National Heart, Lung, and Blood Institute (NHLBI) announced today an advisory recommending that blood transfusions should be continued in order to reduce the rate of strokes (cerebral infarctions or hemorrhages) in children with sickle cell anemia who are at risk of strokes. Strokes occur in approximately 10% of children with sickle cell anemia. These cerebral vascular events can be very debilitating, leading to physical and neuro-psychological impairment which can affect motor skills, school performance, and overall quality of life.

I am a woman age 51. I have always eaten whole grain, no red meat, generally low fat, high fiber diet. My latest blood test came back with very high LDL, and I was shocked (at least my HDL levels were high also). It seems that stress can give you high cholesterol, no matter what you eat. Your own body will make it. So get checked.

The cholesterol information is useful and straight forward. The main solution for high cholesterol seems to be the TLC diet. While those three letters appear many times, nothing tells the reader where to go to find a detailed description of this diet.

I`m using hormones since 1994 and I`m now 63. I can`t think about something more helpful for me than hormones. I hope that this new trend of trying to persuade women to stop taking the hormones by terrifing them with this new research is not a sign of stoping all the research of what is good for women even if it is not perfect yet. Very impressed by this superbly prepared guide that one could save on one’s computer desktop or smartphone for quick reference. Have already highlighted relevant portions on my copy. The "Instead of...Replace with" is a great idea and so is the Sample Reduced Calorie Menus section (was however surprised to not see any mention of egg in any of the breakfast menus). Contained a fair amount of useful information and plenty of encouragement. Some of the example info such as diet or walking plan was particularly useful. A great overview for beginners to weight loss but longer-sufferers such as myself can learn something too.

First, the title of this booklet shouldn’t be "Aim for a Healthy Weight," it should be "Losing weight if you are overweight." There is absolutely no discussion of how to attain a healthy weight if someone is underweight, which occurs less often than obesity in the US but does occur nonetheless. Thus it perpetuates the misconception that thinner is always healthier. Second the advice of substituting certain foods for "healthier" choices often implies that reasonably healthy foods (like whole milk, granola, avocados) are equal culprits in a poor diet as truly empty calories in fast foods or deserts. Please, this is like telling someone who is just leaning how to swim that their form in the butterfly is bad. Concentrate on the basics like not eating fast food, limiting portions, before you worry about if they are drinking 2% milk. Third who wrote the ethnic section? Masala just means a blend of spices, not the calorie or fat content of the dish.

This booklet has a lot of good information to be used for class settings and for the individ. I truly like the BMI chart--for everyone to put it inside their kitchen cabinet so they see it often w/o showing it to the whole world. The best is the Moving Forward--setting goals--rewarding success. Very helpful and inspirational.

We are kicking off an EatWell/Live Well Campaign with members of our local Chamber of Commerce. It is a voluntary program that lasts for eight weeks to encourages participants to loss weight by walking 10,000 steps a day, eat five fruits and vegetables and reduce sugary beverages. Our jurisdiction includes the cities of Barberton and Norton in Northeast Ohio. This interactive website is an excellent resource that we would never be able to afford to create. Thanks. The booklet is very appealing visually and very informative. My clients have expressed special appreciation for the restaurant section. The BMI chart has been an "eye opener" for some. My problem is using the nose and mouth piece and then tried the chin strap with tubing similiar to my oxygen tubing...still can’t use. Part of my problem using the equipment comes from pre-existing illness. I do think it may help if I can use the machine.

This article contains information that has finally answered my long standing puzzle regarding my breathing problem during some of my night sleep. I now understand why my wife would sometimes wake me up in the middle of the night just to tell me that my breathing was irregular, actually stopping for a long time after loud inconsistent snoring, and suddenly jerking out of sleep or sometimes she would wake me up with fear in her face. She suspected beer to be the cause but I ignored it until after reading this article. Three signs fit my condition: loss of memory (complete loss after drinking beer), feeling irritable and mood swings. The suggested apnea diagnosis and treatment methods should be helpful. My only worry is that the CPAC gudgets don’t look comfortable. I must see a physician. Thank you for your article.

Breathing Difficulties Related to Physical Activity for Students With Asthma: Exercise-Induced Asthma

This resource offers tips for addressing exercise-induced asthma by school personnel who are responsible for physical activity and sports programs for students. This tips sheet provides general guidance for responding to and preventing breathing difficulties related to physical activity among students with diagnosed exercise-induced asthma. It is a two-page summary of actions that are described in more detail in Asthma & Physical Activity in the School and in Managing Asthma: A Guide for Schools. It is a handy tool that can be used in conjunction with asthma education programs for school personnel and as a quick reference on coaches’ clipboards.

NHLBI Cardiovascular Disease Thought Leaders Meeting Report: Research Translation, Dissemination, and Application--Moving Toward a New Vision and Strategic Framework

The National Heart, Lung, and Blood Institute is taking a new direction to achieve a higher level of excellence in research translation and dissemination, ensuring that the results of our research truly impact clinical practice and public health. The NHLBI convened a Cardiovascular Disease (CVD) Thought Leaders Meeting to hear their vision of how to achieve greater penetration of NHLBI research results into the marketplace of new scientific ideas, effective medical practice, and consumer health.

The meeting was a rich source of input on three main issues:

1. Developing integrated approaches to CVD risk factor education programs and clinical guidelines
2. Enhancing dissemination of NHLBI-funded research
3. Fostering stakeholder collaboration and knowledge exchange

Other recommendations were:

1. Continue to produce single risk factor clinical practice guidance when warranted and also develop integrated science and clinical recommendations;
2. Work more closely with current stakeholders to help in translating and disseminating our science; and
3. Collaborate more broadly with other agencies and organizations in reaching those at high risk of developing heart disease.

We Can! (Ways to Enhance Children Activity and Nutrition) - Curriculum for Parents and Caregivers

We Can! Energize Our Families: Curriculum for Parents and Caregivers, is a six-lesson curriculum with dynamic activities for parents to encourage a healthy weight in their family. The core concept explored is "energy balance," or the long-term balance between ENERGY IN (calories from food) and ENERGY OUT (calories burned through activity). In a fun and hands-on way, the lessons focus on helping participants learn essential skills that can help their families increase access and availability of healthy foods and make healthful food choices, become more physically active, and reduce recreational screen time. At the end of each lesson, participants are encouraged to try new nutrition and physical activity tips with their families over the next week. Participants later share their experiences trying the new behavior with the group.

The full curriculum is also available in Appendix A of the We Can! Energize Our Community Toolkit for Action.

The DASH study results should be the basis for the way everyone eats. Unless you have a specific health issue that prevents you from using the the "DASH diet" you should be eating this way, this is how you should teach you children to eat, this is how you should feed your family. If every one in the U.S. Ate this way we would solve 8 of the ten top health issues in this country. The cost is nothing compared to the price we pay treating the conditions that are caused by poor dietary habits.

I have uncontrolled high BP. Still don’t have the right drugs for my pressure that goes as high as 200/130. Yet still I don’t have a headache or any bad symptoms. I have started Mrs Dash diet over a two wk period and my pressure is 140/90. I am going to continue this until I see a difference.

Jim Fixx died in 1984 (his arteries were plugged with too much cholesterol). His book The Complete Book of Running was a best seller. He thought that being super fit he could eat whatever he wanted. I’ve lost a ton of weight with another diet but this is better for BP and balanced nutrition. No need to take vitamins in an artificial form. The PDF is a million dollars worth of research diet book for FREE!

I have HBP and high cholesterol in my family. My father has had a serious heart attack and my uncle died from a heart attack. My doctor just tells me to eat right and excercise, but nothing more specific. This PDF download is great! My numbers are in a good place now and I am maintaining my weight. Would like to lose about 5 - 8% more weight, but need to excercise more. Thanks for the recipes!

Information is easy to understand, the guidelines are simple to follow. If everyone followed these basic concepts, obesity, high blood pressure, diabetes, and heart disease would be greatly reduced.

Am 63. In seven weeks, lost 13 pounds, lowered BP significantly. Overall cholesterol went from 250 to 198; LDL from 155 to 125 and Triglycerides from 191 to 77. With only one kidney I was terrified of my doctor recommended medicine. Now she is almost as delighted as I am. Oh, and don’t forget the exercise which I do most days for thirty minutes. Also am never hungry and am enjoying the summer fresh veggies and fruits.

I give the NON-EXISTENT Spanish version 0 stars. I give the English version 5 stars but it is useless for all my Spanish-speaking patients with limited English. I can’t find a Spanish-language alternative to this publication. None of them provide such a comprehensive instruction on how to implement the DASH diet. PLEASE, I BEG OF YOU, TRANSLATE IT INTO SPANISH!! (would it help if I beg some more???)

The menu suggestions are way too complicated -- they seem to have been written for the 1950’s, with a mommy housewife in every suburban ranch house. 21st century folks often don’t have the time to make a dinner of 10 ingredients! A simpler, quick- or no-cooking version is needed, or seals of approval for frozen dinners that meet the DASH standards.

Like most, I’m trying to eat (and drink) better, including drinking more water. I’ve been buying gallons of "purified drinking water" from the local market, only to discover they have sodium bicarbonate and sodium sulfate in them "for flavor." I’ve switched to distilled water.

Thanks for the Dash eating plan. Doctors only want to medicate, without a search for cause. I’m convinced that even with hereditary factors, good food, reasonable exercise and minimal stress will lower blood pressure. Now I’m convinced based on the Dash study. I’ve lost and kept off 148 pounds following the DASH plan. I started out just adding a vegetable or two at meals. Then my husband joined in. Soon we were eating less salt & my blood pressure gradually went down. We both walk for exercise every day. My cholesterol went down to normal too. It took me five years to lose the weight, but I feel terrific that with DASH as a template, the weight loss is permanent for a longer and much healthier life. Thank you so much!

After reading reports on USDA Food Plans and costs, and serving information on My Pyramid, what I need it a menu planner with recipes and a shopping list, interactive so I can plug in the number of members in my family and their ages, so it fits us. The potassium to sodium ratio is the key. Reducing salt is very important, but increasing potassium should be emphasized also. I have switched to eating more fresh fruits and vegetables and stay away from processed food as much as possible. I am now off the beta-blocker drugs. Why hasn’t the food industry cut back on salt? One can of Hormel chili has 2200 mg of sodium. You would think they don’t care about our health.

I think it would be reasonable to ask pts or clients to spend $3.50 for an excellent resource. If we all did some soul searching, we probably spend that much on a coffee or bag of potato chips, pretzels, etc. The price of a single book is very reasonable, in my opinion and if a pt is motivated, it is not too much to ask.

Following the DASH diet and lifestyle produced many positive health benefits. It lowered my blood pressure, reduced my cholesterol levels and my waist and weight went down as well. This is a common sense approach for anyone who wants to lead a healthy lifestyle. If you want to increase the quality and quantity of your lifespan then a sensible dietary approach like DASH is the way to do it!

I started the DASH plan on January 1,2007. I browsed the site, checked my weight and BMI with the easy charts on the site. I found the easy eating plans and the suggestions for planning my meals so easy to follow. I printed several of the pages from the PDF file after looking through the whole booklet of information. I had my annual check up yesterday and my blood pressure was down in the normal range, and I have lost 9 pounds! I am so happy I found your website and have shared it with others that are interested in knowing exactly what is good to eat and how to watch for that hidden sodium in so much of the food we eat. My doctor said I was the healthiest 65 year old woman she has seen! I combined the eating plan with a 2 mile walk every day, and I feel great because I found what works!

letters spacing words are all off, making it irritating and bothersome for the patients. It’s not teacher/user friendly The premise is excellent, the information would be vital, it’s simply not user friendly

I have used the old format edition in the past and people have loved it. However, the new edition is too expensive to hand out on a large scale such as community or worksite health.

Great Resource! It is too expensive to purchase for use in community nutrition where we depend largely on take home materials to reinforce concepts that have been taught. Hope you can come up with a PDF downloadable form that will be more economical.

Great item - too expensive!!! I have used this in residency for the past several years and have promoted it. I think its great information for patients BUT too expensive for me to purchase and hand out.
