Future Research Directions in Chronic Obstructive Pulmonary Disease - NHLBI Workshop SummaryBetween 3 and 7 million Americans are currently diagnosed with chronic obstructive pulmonary disease (COPD), and the true prevalence is probably greater than 16 million (1). Many of these individuals suffer years of progressive discomfort and disability. With the number of deaths per year attributed to this disease at approximately 100,000 and increasing, COPD is now the fourth leading cause of death in this country (2) and is expected to be third by the year 2020. Cigarette smoking is firmly established as the major cause of COPD, but approximately one-quarter of Americans continue to smoke, despite aggressive smoking prevention and cessation efforts. Better means are clearly needed for the prevention and treatment of COPD, and more scientific research is needed to enable improvements in its clinical management. Acute Lung Injury: Future Research Directions, Workshop Summary, January 21-22, 2002Acute lung injury (ALI) and its more severe form, the acute respiratory distress syndrome (ARDS), are syndromes of acute respiratory failure that result from acute pulmonary edema and inflammation. The development of ALI/ARDS is associated with several clinical disorders including direct pulmonary injury from pneumonia and aspiration as well as indirect pulmonary injury from trauma, sepsis, and other disorders such as acute pancreatitis and drug overdose. Although mortality from ALI/ARDS has decreased in the last decade, it remains high. Despite two major advances in treatment, low VT ventilation for ALI/ARDS and activated protein C for severe sepsis (the leading cause of ALI/ARDS), additional research is needed to develop specific treatments and improve understanding of the pathogenesis of these syndromes. The NHLBI convened a working group to develop specific recommendations for future ALI/ARDS research. Improved understanding of disease heterogeneity through use of evolving biologic, genomic, and genetic approaches should provide major new insights into pathogenesis of ALI. Cellular and molecular methods combined with animal and clinical studies should lead to further progress in the detection and treatment of this complex disease. ATP III Update 2004: Implications of Recent Clinical Trials for the ATP III GuidelinesThe Adult Treatment Panel III (ATP III) of the National Cholesterol Education Program issued an evidence-based set of guidelines on cholesterol management in 2001 (Executive Summary published in JAMA, 2001;285:2486-2497). Since the publication of ATP III, 5 major clinical trials of statin therapy with clinical end points have been published. These trials addressed issues that were not examined in previous clinical trials of cholesterol-lowering therapy. Clinical Alert: Transfusions Should Continue in Children with SCD who are at Risk of StrokesThe National Heart, Lung, and Blood Institute (NHLBI) announced today an advisory recommending that blood transfusions should be continued in order to reduce the rate of strokes (cerebral infarctions or hemorrhages) in children with sickle cell anemia who are at risk of strokes. Strokes occur in approximately 10% of children with sickle cell anemia. These cerebral vascular events can be very debilitating, leading to physical and neuro-psychological impairment which can affect motor skills, school performance, and overall quality of life.
