NAME AND ADDRESS OF OFFERORS: To all recipients of RFP No. NHLBI-HC-98-36

Paragraph A. subparagraph 5. is amended to read as follows:

5. Perform readings and information abstractions of CT measurements on approximately 3,152 studies during the pilot test and 12-month examination period.

[Note: It is expected that 3,890 CARDIA participants will undergo the Year 15 examination and that 80% (n=3,112) will undergo CT examinations during the 12-month examination period, from June 1, 2000 to May 31, 2001. Plan to reread 5% of all studies by a second reader (n=156) for quality control purposes. Plan for a pilot study to include approximately 10 studies per site with rereadings by the same and a second reader to estimate intra- and inter-reader variability. Thus, the number of pilot studies would be aproximately 40 (reader A) + 40 (reader A again) + 40 (reader B). Offerors are also free to propose alternative strategies for the pilot study.]

Please acknowledge receipt of amendment 1 (ONE) by including it with your original business proposal. The due date for receipt of proposals remains unchanged by this amendment.

Sincerely,
/s/
William M. Stevens
Contracting Officer
